The St. Johns County Employee Connection FYI from Personnel Services 2021 Wellness Premium Credit Employees and spouses enrolled in the SJC medical plan will once again be eligible for a premium credit in 2021 by completing their annual wellness exam between October 1, 2019 and September 30, 2020. Enrolled participants who have completed their wellness exam at their physician’s office within the program dates will be automatically reported by Florida Blue. No documentation of completion is needed for those enrolled in the medical plan, but you can contact the Benefits Team at 904-209-0635, option 5 or bccbenefits@sjcfl.us to request a verification form. Participation in this program is voluntary. Medcom & Florida Blue Onsite Visit Have questions or need help setting up your online account(s)? Medcom will be onsite to answer questions about IRS substantiation and FSA/HRA/DCA spending account balances. Florida Blue will be onsite to help with provider searches and claims assistance. Long Term Care Insurance | Enrollment SJC is offering an enrollment specifically for Long-Term Care (LTC) Insurance now through March 13. LTC is a new voluntary benefit for benefit eligible employees and their spouses offered through Mutual of Omaha. This enrollment window includes reduced underwriting and limited health questions. You’ll setup direct payment with Mutual of Omaha once approved and coverage is 100% portable. Visit the LTC Enrollment Website to register for an informational webinar on Tuesday, March 3, 10:00 am & 2:00 pm OR Wednesday, March 4, 2:00 pm ET. Webinars will consist of a 30-minute presentation followed by Q&A. W-2 & 1095-C Tax Forms 2019 W-2 Forms will be issued by Friday, January 31st. As in previous years, W-2 Forms will be distributed through department timekeepers. Any W-2 Forms not picked up will be mailed to the employee address on file. Employees eligible for medical insurance coverage offered by St Johns County will receive a 1095-C form in the mail beginning March 2nd. Although the 1095-C is not needed to complete your taxes for 2019, employers and insurance providers are still required to furnish these forms to employees. Vaccination Clinics & Flu Shots Vaccination clinics return in February for employees of St. Johns County enrolled on the County’s medical insurance plan. Vaccines will be administered by CVS Health on a first-come, first-served basis for employees only. Shingles, Hepatitis A, Hepatitis B, Tdap, MMR, Pneumonia, and Flu Shots will be available.
